About the role

We are seeking a Part-time Registered Nurse for our Nurse Family Partnership program in York. This role is an opportunity to learn, grow, and contribute as part of a dedicated team committed to trustworthy service and care.

Responsibilities

  • Provide comprehensive nursing, education, and support services for first-time mothers and their families in accordance with NFP program goals, objectives, and model elements to improve pregnancy outcomes, provide parenting education, and promote life-course goal achievement.
  • Develop and maintain therapeutic relationships with patients, establishing appropriate boundaries through home visitations.
  • Assess physical, emotional, social, and environmental needs of women and their families; plan interventions across the six established NFP domains.
  • Adapt NFP guidelines to meet individual patient needs without compromising program content.
  • Evaluate patient needs and behaviors with cultural, racial, and other differences in mind to ensure effective care delivery.
  • Provide appropriate nursing care during pregnancy and postpartum to mother and baby based on NFP checklists and clinical practice.
  • Assist patients in identifying realistic short- and long-term goals for themselves and their babies.
  • Make referrals to community resources and coordinate care effectively.
  • Maintain contact with patients to sustain the therapeutic NFP relationship throughout the program duration.
  • Complete required documentation accurately and in a timely manner.
  • Follow NFP guidelines to function as a productive team member, organizing time and schedule efficiently.
  • Attend and actively participate in all required training, education, and meetings.
  • Ensure data entry is completed accurately and on time.
  • Carry a caseload of up to 25 patients (1.0 FTE equivalent).
  • Perform other duties as assigned.

Requirements

  • Active RN license and a Bachelor of Science in Nursing (BSN).
  • Current CPR and Narcan certification.
  • Knowledge of computer software applications (Microsoft Office and clinical reporting software).
  • Strong grammar, spelling, and punctuation skills for professional correspondence.
  • Basic arithmetic skills for calculations, balancing, and reconciling figures.
  • Excellent customer service and strong analytical skills.
  • Proficiency in operating office equipment and keyboarding.
  • Ability to critically think, evaluate arguments, and make logical connections.
  • Ability to travel between work sites and home visitations; valid driver’s license required.
  • Ability to work in and respond to a demanding environment.
  • Strong communication skills to engage with individuals of all backgrounds in a caring and supportive manner.
  • Ability to work efficiently with strong attention to detail and maintain effective working relationships with patients, employees, and the public.

Physical Demands

  • Frequent travel between work sites and home visits, with exposure to varying weather conditions.
  • Subject to unpredictable situations, including exposure to pets, pests, weapons, drugs, or poor home maintenance.
  • Constant contact with patients and others in their homes under diverse circumstances.
  • Occasional exposure to disease, infection, virus, blood, and bodily fluids.
  • Frequent and prolonged computer and phone use, involving repetitive typing and hand motion.
  • Requires vision and hearing within normal range.
  • Frequent bending, standing, stair climbing, and walking.
  • Occasional stooping, crouching, or kneeling.
  • Frequent turning, reaching, and use of common PPE/safety equipment.
  • Ability to manage competing demands, frequent changes, delays, or unexpected events.
