About the role

Coordinate care for pediatric patients with cardiac conditions across all stages, from referral to follow-up. Collaborate with cardiologists, surgeons, and interdisciplinary teams to ensure seamless patient management. Provide education, support, and advocacy to patients and families.

• Ensure timely scheduling of appointments and assist in managing complex care plans.

• Mentor peers and promote a patient-centered, compassionate care environment.

Qualifications & Skills

• Registered Nurse licensure with relevant pediatric or cardiology experience preferred.
• Strong communication, collaboration, and advocacy skills.
• Ability to manage complex care coordination and provide patient education.
• Commitment to quality improvement and continuous learning in a healthcare setting.
