Registered Nurse (Multiple Positions)

About the role

Participate in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care and update changes as necessary.

Responsibilities

• Attend Interdisciplinary Team (IDT) care plan meetings and Participant/Family meetings as applicable.
• May perform assessments in a variety of settings including participant's place of residence, PACE Clinic and/or Adult Day Center.
• Develop and provide education to participants, caregivers, and associates as necessary including end of life discussions.
• May require travel to participant's place of residence, long term care and assisted living facilities.
• Assess participants in their homes for home care needed to support their independence living in the community.
