Key Responsibilities
- Implementing and monitoring patient care plans
- Maintaining accurate and timely documentation of patient conditions
- Communicating patient conditions to interdisciplinary team members
- Collaborating with physicians and other healthcare providers to coordinate care
- Serving as a primary point of contact for patients and families
- Assessing and coordinating discharge planning needs
- Current RN licensure from the Wisconsin Board of Nursing or applicable state license
- American Heart Association or Red Cross Basic Life Support (BLS) certification within 30 days of hire
- Advanced Life Support (ACLS) certification within 6 months of hire
- Diploma from an accredited nursing program or equivalent professional licensure
