Responsibilities Lead the delivery of patient-centered care and coordinate multidisciplinary teams for a seamless treatment experience.Monitor and interpret patient conditions in real-time, ensuring timely medical interventions.Advocate for patients and families through effective discharge planning and safe transition from hospital to home. Qualifications & Skills Licensed Registered Nurse credential from the Indiana Board of Nursing.BLS Provider certification within 30 days of hire, accepted from American Heart Association or Red Cross.Diploma from an accredited nursing school or equivalent professional licensure.
