About the Role
Responsible for delivering personalized care to renal and diabetic patients in a Med Surg Renal Diabetic department.
Responsibilities

- Coordinate multidisciplinary care teams to provide seamless, holistic patient care.
- Accurately translate physician and nursing orders into clinical actions, maintaining safety and care standards.
- Develop comprehensive discharge strategies to empower patients and reduce readmissions.
Requirements
- Current RN licensure from the Indiana Board of Nursing.
- BLS Provider certification within 1 month of hire, accepted from American Heart Association or Red Cross.
- Diploma from an accredited nursing school or equivalent professional licensure.
