Founded in 1999, Progressive Community Health Centers is an independent non-profit organization that operates four clinic locations in Milwaukee, Wisconsin. More than 100 employees support our mission to improve the health and quality of life of the community by providing culturally competent services that address identified needs. We provide high-quality, comprehensive care to roughly 16,000 patients annually, the majority of whom are low-income. No one is ever turned away from our clinics based on insurance status or ability to pay.

About the role

The RN/LPN works collaboratively as an active member of the interprofessional team to provide therapeutic patient- and family-centered care. The RN/LPN systematically and continuously collects and assesses data related to patient health status to develop, execute, and evaluate the plan of care. The RN/LPN utilizes evidence to drive clinical decisions and practice related to individual and population health. The RN/LPN establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The RN/LPN considers factors related to safety, effectiveness, and fiscal accountability in planning and delivering care. The RN/LPN participates in continuous quality improvement initiatives and assumes responsibility for professional development.

Responsibilities

• Provides coordinated, patient- and family-centered nursing care by actively collaborating with the interprofessional team to meet care needs across settings.
• Telephone and MyChart triage: According to the chief complaint, assess the severity of the patient’s symptoms and guide the patient to the appropriate level of care (scheduled visit, same-day visit, urgent care, or emergency care).

Requirements

• Active Wisconsin RN or LPN license.
• Experience in a primary care or community health setting preferred.
