Responsibilities

  • Completes patient assessment and reassessment as clinically indicated.
  • Documents detailed information incorporating the plan of care, interventions and outcomes reflecting the patient's physical, mental, and emotional status.
  • Ensures appropriate documentation based on the level of care the patient is receiving to include transfer/discharge documentation as indicated.
  • Fulfills the five Rights of Medication Administration. Verbalizes to patients/family/significant others the medication they are receiving.
  • Affirms the patient prior to administering medications noting allergies, parameters, potential side effects, effectiveness and intervening as needed. Reassesses and documents response according to hospital policy.
  • Collaborates with patients, family/significant others, peers, physicians, and other members of the interdisciplinary healthcare team in the delivery of healthcare services to patients.
  • Maintains accountability for care delegated.
  • Works to promote a culture of safety: Assesses patients for potential safety risks. Adheres to hospital policies and procedures regarding patient safety. Initiates interventions related to the risk factors identified.
  • Conducts bedside shift report and bedside rounds with peer on assigned patients at change of shift.
  • Demonstrates quality by documenting the application of the nursing process in a responsible, accountable, and ethical manner.
  • Safeguards patient hand-off as appropriate when leaving for breaks.
  • Assesses learning needs of patient/family/significant other. Plans and provides appropriate instruction using applicable teaching methods. Assesses and documents patient/family/significant other response to teaching.
  • Cares for specific patient populations, despite location, as necessary.
  • Delivers Patient Centered Care: Embraces the cultural diversity and difference of patients and team members. Respects the dignity of all individuals through the maintenance of the caring relationship. Maintains open lines of communication inclusive of actively listening.
  • Meets accreditation, regulatory, and compliance responsibilities related to the position.
  • Works to promote a culture of learning and growth: Seeks experiences and formal and independent learning activities to maintain and develop clinical and professional skills and knowledge. Teaches others to succeed through mentorship.
  • Performs other duties as assigned within the state nursing scope of practice.

Requirements

  • Current and unobstructed New York State Registered Nurse (NY Hospitals) license.
  • Current and unobstructed Connecticut Registered Nurse License (CT Hospitals).
  • Preferred certifications: American Nurses Association endorsed Certifications or Academy of Medical-Surgical Nurses Certification.
  • Training Units Required / Preferred:
    • BLS All Nursing Care Areas Required upon hire.
    • ACLS ED, ICU, CCU, SDU PCU, PACU, Cath Lab, GI, LDRP, IR, Float Pool- specific to individuals' competency and location floated.
    • PALS ED, PACU, Pediatric Areas (Either PALS or ENPC) Required by end of orientation period.
    • NRP MCH, NICU, LDRP, DR Required by end of orientation period.
    • EFM LDRP Within 1 year of hire.
    • TNCC ED (Required Danbury, Norwalk), ICU(Preferred, But May Have TCAR) PACU(DH � Preferred) Within 1 year of hire.
    • ENPC ED RN (Either PALS or ENPC) Within 1 year of hire.
    • TCAR ICU (VBMC- Preferred) ICU/PACU (NH) Within 1 year of hire.
    • CPI or other de-escalation technique training Behavioral Health, ED (All hospitals) Float Pool Putnam 6W, Float Pool (Norwalk) ICU, PCU, 9 tower (DH) Within 6 months of hire.
    • S.T.A.B.L.E OB(NDH Only) Within 6 months of hire.
    • Chemotherapy Certification Inpatient Oncology Within 1 year of hire.
    • ED: Emergency Department.
    • NICU: Neonatal Intensive Care Unit.
    • ICU: Intensive Care Unit.
    • MCH: Maternal Child Health Unit.
    • CCU: Critical Care Unit.
    • LDRP: Labor Delivery Recovery Unit.
    • OR: Operating Room.
    • DR: Delivery Room.
    • PCsecU: Progressive Care Unit.
    • M/S: Medical Surgical Unit.
    • SDU � Step Down Unit.
    • CPI: Crisis Prevention Intervention.
    • PACU: Post Anesthesia Care Unit.
    • IR: Interventional Radiology.
