About the Role

The Registered Nurse II provides patient triage in person and telephonically, supporting providers, health centers, support staff, and other site or program-based RNs. This role involves patient care, education, consultation, and collaboration with medical providers to ensure optimal health for patient populations.

Responsibilities

  • Make quick decisions, multitask, and handle stress while demonstrating strong written and verbal communication skills.
  • Assist in planning and coordinating agency and community responses to disease outbreaks; participate in public health emergency response teams as needed.
  • Record patients' medical histories, perform diagnostic tests, administer medications, and develop patient care plans in conjunction with the primary care team.
  • Conduct co-visits and assist providers during examinations and procedures, operating and monitoring medical equipment as needed.
  • Support and enhance patients' physical and emotional stability; explain the nature of illness, plan of care, procedures, and treatments to patients, families, and significant others.
  • Perform nursing procedures and designated medical treatments within scope of authority; observe and assess patients' conditions and behavior; report significant changes to physicians.
  • Triage patients experiencing symptoms and relay their condition to the care team.
  • Participate in coordinated care for patients seeking care via telephone and MyChart messages; follow up with patients and ensure communication with the primary care team.
  • Promote independence by teaching patients and families to understand medical conditions, medications, and self-care skills.
  • Present patient and family teaching programs based on identified health care goals.
  • Coordinate with the Care Transitions team for all hospitalized patients to ensure timely follow-up and return to primary care.
  • Use clinician-approved standardized procedures to order labs, preventive screenings, and refill routine medications.
  • Coordinate care closely with other team members to identify patient self-care goals and ensure medical and psychosocial needs are met; facilitate follow-up care as needed.
  • Act as a resource for support staff.
  • Maintain absolute confidentiality of work-related issues, patient records, personnel records, and County information in compliance with the Privacy Act of 1974 and HIPAA policies.
