Registered Nurse II, Primary Care at Home
Duties / Responsibilities
- Provide age-appropriate, patient-centered care to pediatric through geriatric populations in both virtual and in-person settings.
- Achieve optimal patient outcomes through the Nursing Process, assessing, planning, implementing, and evaluating care.
- Deliver services such as wellness visits, care management, point-of-care testing, and triaging patients.
- Collaborate closely with providers to support examinations, procedures, and efficient clinic operations.
- Serve as a key liaison among patients, providers, and staff, ensuring clear communication, coordinating workflow, and responding to urgent situations.
- Support staff development through training and mentorship.
- Participate in quality and compliance initiatives and contribute to process improvement efforts.
Requirements
- A graduate of a state-approved or National League of Nursing-accredited School of Nursing.
- Registered Nurse licensure or eligibility for licensure in Delaware, Maryland, Pennsylvania, or New Jersey.
- Two years' experience as a Registered Nurse, with physician office experience preferred.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (AND) will be considered with an agreement of completion of BSN in three years.
- Basic Life Support (BLS) certification required.
