Registered Nurse II, Primary Care at Home

Duties / Responsibilities

• Provide age-appropriate, patient-centered care to pediatric through geriatric populations in both virtual and in-person settings.
• Achieve optimal patient outcomes through the Nursing Process, assessing, planning, implementing, and evaluating care.
• Deliver services such as wellness visits, care management, point-of-care testing, and triaging patients.
• Collaborate closely with providers to support examinations, procedures, and efficient clinic operations.
• Serve as a key liaison among patients, providers, and staff, ensuring clear communication, coordinating workflow, and responding to urgent situations.
• Support staff development through training and mentorship.
• Participate in quality and compliance initiatives and contribute to process improvement efforts.

Requirements

• A graduate of a state-approved or National League of Nursing-accredited School of Nursing.
• Registered Nurse licensure or eligibility for licensure in Delaware, Maryland, Pennsylvania, or New Jersey.
• Two years' experience as a Registered Nurse, with physician office experience preferred.
• Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (AND) will be considered with an agreement of completion of BSN in three years.
• Basic Life Support (BLS) certification required.
