About the Role

As a Hospice Case Manager, you will collaborate with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role involving travel within Boise, ID, and surrounding regions to deliver in-home care from admission to discharge, ensuring each patient receives the highest standard of care aligned with their needs.

Schedule

Full-time: Monday–Wednesday, 08:30–17:00. Includes Administrator On Call (Monday–Thursday, 17:00–08:30) one week per month.

Responsibilities

• Manage individualized care plans for a designated caseload of adult patients (occasional pediatric training available).
• Travel within Boise and surrounding regions to provide in-home care.
• Daily visit requirements below state and national averages.

Pay

• Paid hourly, including drive time.
• Mileage reimbursement for travel within Boise and surrounding regions.
• On-call pay and applicable differential pay.
• Sign-on bonus.

Requirements

• ASN or BSN in Nursing.
• 2 years of RN experience; hospice case management experience preferred.
• Current RN license in Idaho and Basic Life Support (BLS) certification.
• Valid driver’s license and proof of auto insurance.
