Responsibilities
- Deliver compassionate, patient-centered care using therapeutic interventions, evidence-based practices, and interdisciplinary team coordination.
- Develop skills in wound care, medication administration, IV monitoring, and caregiver education.
- Ensure patients' comfort, dignity, and quality of life during end-of-life care.
Qualifications & Skills
- Current RN license
- BSN degree (or obtaining within four years if less than 12 months of experience)
- BLS certification
- Valid driver’s license with reliable transportation
- Effective communication skills
- Physical ability to perform patient mobility and equipment handling tasks
Key Duties
- Manage autonomous patient care
- Collaborate with healthcare team
- Educate patients and families
- Adapt to diverse clinical needs in a home environment
