Registered Nurse, Home Infusion - Registry

About the role

The Home Infusion Registered Nurse at UChicago Medicine provides intravenous medication administration for patients in both the home and infusion suite settings. The professional nurse plans, coordinates, and performs home infusion services with a focus on community health education and experience. They work cooperatively with pharmacy staff to establish priorities and goals of patient care.

Responsibilities

• Administers medications and treatments as prescribed by the physician.
• Maintains close monitoring of patient responses to interventions.
• Assesses and monitors patients for anaphylaxis and intervenes as necessary during home infusion skilled nursing services.
• Trains patients on home infusion therapy self-administration and provides direct nursing care in the patient's home.
• Regularly reassesses patient nursing needs.
• Completes an initial assessment of the patient and family to determine home care needs.
• Conducts a comprehensive physical assessment and history of current and previous illnesses.
• Develops a care plan that includes established goals, therapeutic, preventive, and nursing actions, as well as psychosocial support as needed.
• Involves the patient and family in the planning process.
• Documents all required patient care, including the nursing plan of care and reports.
• Performs other duties as assigned.

Requirements

• One to two years of acute care experience in an institutional setting.
• A valid driver’s license and current automobile insurance.
• Current state of Illinois nursing license.
• BLS (Basic Life Support), ACLS (Advanced Cardiac Life Support), Chemotherapy & Immunotherapy Class – Adult Hematology Oncology, and Fundamentals ONS Chemotherapy/ Immunotherapy Certificate within 12 months of hire.
• One year of home health care experience.
• Oncology experience.
• Infusion experience.

Qualifications
