Registered Nurse - Home Infusion Coordinator
About the role
The UVMMC Home Infusion Nurse Coordinator (HINC) is responsible for assuring appropriate transition of patients to the home infusion service by creating, revising, and assessing the patient plan of care relative to identified criteria. The HINC teaches patients, caregivers, and home health care nurses to administer infusion therapy in the home and makes recommendations regarding safety to MD and inpatient Case Manager.
The HINC coordinates with health care team members to assure timely progression of patients across the continuum of care and works with home health care professionals to ensure all involved are prepared to participate in home infusion therapy for the patient. The HINC maintains contact with patients, caregivers, and health care professionals through assessments via phone weekly or as needed.
The HINC supports quality improvement through evaluation of variances to identified protocols, maintains quality data systems, event reports, and identifies and participates in quality projects. The HINC also maintains inventory particular to patient needs.

Responsibilities
- Create, revise, and assess patient plans of care for home infusion therapy.
- Teach patients, caregivers, and home health care nurses to administer infusion therapy safely.
- Make safety recommendations to physicians and inpatient case managers.
- Coordinate with health care team members to ensure timely patient progression across the continuum of care.
- Maintain regular contact with patients, caregivers, and health care professionals through phone assessments.
- Support quality improvement by evaluating variances to protocols and maintaining quality data systems.
- Participate in quality projects and manage inventory specific to patient needs.
Requirements
- Current RN licensure or compact licensure recognized by the State of Vermont.
- ADN required; BSN preferred.
- Two to five years of related clinical experience, such as case management, home health, or public health nursing, or a combination of education and experience.
