Registered Nurse - Home Healthcare

About the role

This position offers an opportunity for a Registered Nurse (RN) to deliver personalized home healthcare, assessing, planning, and implementing patient care under the supervision of a clinical manager.

Responsibilities

• Evaluate patient needs and develop customized care plans.
• Educate patients and families on disease management, procedures, and medications.
• Manage various clinical needs including wound care, tracheostomy, G-tubes, catheters, ventilators, and medication administration.
• Maintain accurate electronic charting and documentation.
• Build long-term, one-on-one relationships with patients, helping them regain health and independence.

Qualifications

• Open to new graduates, with support through a Novice Nurse Program including lab and hands-on training.
• Strong clinical assessment, care planning, and patient education skills.
• Ability to manage diverse cases such as pediatric, adult, and higher acuity cases if desired.
• Excellent communication skills and ability to work independently and as part of a team.
