Responsibilities

  • Manage an assigned patient caseload daily, ensuring timely care and documentation to meet patient needs/services as prescribed in the established plan of care and per MD orders.
  • Collect data pertinent to the healthcare consumer's health or situation from patient assessments in the home setting, telephonic contacts, and/or related EMR information.
  • Analyze data to determine issues and available resources for effective resolution.
  • Identify patients at high risk for re-hospitalization, falls, and/or high utilization of resources; provide additional intervention and case conferencing to mitigate risk and ensure high-quality outcomes.
  • Prioritize multi-issue situations based on the criticality of patient health and welfare, patient/staff safety, and impact on timely services.
  • Identify barriers to situation/complaint resolution (e.g., language, literacy, psychological) and follow up with appropriate resources or leadership.
  • Develop and oversee a plan of care, effectively communicating it to involved parties to support quality patient outcomes and satisfaction.
  • Implement the plan and direct care coordination using a multidisciplinary clinical team approach, including supervision of LPNs and/or HHAs.
  • Perform ordered treatments, therapies, and/or procedures in accordance with the State Nurse Practice Act, Home Health Conditions of Participation, JCAHO, and agency policies.
  • Evaluate progress toward attainment of patient-centered goals.

Physical Requirements

  • Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
  • Must be able to lift and support up to 35 pounds when handling patients, medical equipment, and supplies.
  • Must speak English clearly and understandably.
  • Intact senses of sight, hearing, smell, and touch; finger dexterity.
  • Critical thinking and ability to concentrate.
  • Must respond quickly to changes in patient conditions.
  • Physical Abilities Testing required.

Qualifications

  • Graduate from an accredited School of Nursing; BSN preferred.
  • BLS certification required per policy guidelines.
  • Current RN license or temporary license as a Registered Nurse Petitioner in the state of work and residence, or if declaring a National License Compact (NLC) state as primary residence, meet licensure requirements in your home state. For Non-National License Compact states, a current RN license or temporary license as a Registered Nurse Petitioner is required in the state where the RN works.
  • Additional education, training, certifications, or experience may be required within the department by the nurse leader.
