Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
• Coordinate the total plan of care with other health care professionals involved in care.
• Maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources.
• Perform all duties within a Point of Care setting.

Requirements

• Graduate from a recognized, accredited school of nursing.
• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNLC Multistate Licensure (upon hire).
• CPR - Cardiac Pulmonary Resuscitation certification (upon hire).
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation (upon hire).
