Position Overview

Seeking a compassionate and experienced Registered Nurse (RN) to provide hospice case management services throughout the Memphis metro area. This role focuses on delivering patient-centered end-of-life care, coordinating services, and supporting both patients and families. The ideal candidate has hospice experience, is proficient in HomeCare HomeBase, and demonstrates flexibility and a strong commitment to doing what matters most for patients.

Schedule & Work Details

  • Location: Shelby, Fayette, and Tipton Counties (Memphis metro)
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 32 hours
  • On-call/Weekend Requirement: None
  • Pay Rate: $51.00 to $53.00 per hour
  • Mileage Reimbursement: Yes, between patient visits from first patient to last patient

Key Responsibilities

  • Provide hospice RN case management for assigned patients
  • Perform patient assessments and develop individualized care plans
  • Complete hospice admissions, routine visits, and occasional death visits as needed
  • Prepare for and participate in Interdisciplinary Team (IDT) meetings
  • Coordinate care with physicians, social workers, chaplains, and other team members
  • Educate patients and families on disease progression, symptom management, and end-of-life care
  • Maintain accurate and timely documentation using HomeCare HomeBase
  • Deliver compassionate care while ensuring compliance with hospice standards and regulations

Required Qualifications

  • Active Registered Nurse (RN) license in Tennessee or compact state
  • Hospice experience
  • Experience with HomeCare HomeBase (HCHB)
  • Strong clinical assessment and case management skills
  • Ability to work independently and manage a caseload
