Position Overview

Seeking a skilled and experienced Registered Nurse to provide high-quality home health care to patients in Howard and Carroll County. This field-based role focuses on delivering individualized care, managing complex clinical needs, and supporting patients in a home setting to improve health outcomes and quality of life.

Schedule & Work Details

  • Location: Howard and Carroll County coverage area
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Guarantee: 32 hours
  • On-Call/Weekend Requirement: On-call required
  • Mileage Reimbursement: Yes, between patient visits (first to last patient)
  • Dress Code: Navy blue scrubs

Pay Rate: $51 to $53 per hour

Responsibilities

  • Provide skilled nursing care to patients in their homes in accordance with physician orders and plan of care
  • Perform patient assessments, identify changes in condition, and adjust care plans as needed
  • Administer medications, treatments, and specialized procedures
  • Manage complex clinical tasks including central line care, wound care, wound vacs, and catheter care (foley and suprapubic)
  • Perform venipuncture and specimen collection
  • Document patient care accurately and timely in accordance with agency and regulatory standards
  • Collaborate with physicians and interdisciplinary care team to coordinate patient care
  • Educate patients and caregivers on disease management, medications, and home care techniques

Requirements

  • Active Registered Nurse (RN) license in the appropriate state
  • Prior home health experience required
  • Strong clinical assessment and documentation skills
  • Ability to work independently and manage a field-based schedule

Preferred Qualifications

  • Experience with pleurex drains
  • Experience with central line care
  • Proficiency in wound care and wound vac management
  • Experience with catheter insertion (foley and suprapubic)
  • Strong venipuncture skills
  • Excellent time management and communication skills
  • Experience with Homecare Homebase (HCHB) is a plus
