Registered Nurse, Home Health - Corpus Christi, TX

About the role

We are seeking a Registered Nurse, Home Health, to perform comprehensive nursing assessments, develop and evaluate care plans, and deliver skilled nursing services such as medication administration, wound care, and patient education.

Responsibilities

• Assessing patient conditions and revising care plans accordingly
• Providing health teaching and promoting patient independence
• Coordinating care services and scheduling with the healthcare team
• Monitoring patient progress and communicating with physicians and team members
• Documenting all interactions and care activities accurately
• Supervising home health aides and participating in case conferences

Qualifications

• Graduate of an accredited nursing program
• Current unrestricted Texas RN license or compact license
• BLS certification (or ability to obtain within 30 days)
• At least 1 year of home health or related experience, wound care knowledge
• Proficiency in Microsoft Office and strong documentation skills
• Reliable transportation for patient visits

Preferred

• Bilingual in Spanish
• OASIS experience

Physical requirements

• Ability to lift and transfer patients

Schedule

• Monday to Friday with rotating on-call responsibilities
• Traveling to patient homes within the service area
