Registered Nurse - Home Health

About the Role

Provide skilled nursing and rehabilitative services to clients in their homes, focusing on comprehensive care and independent clinical management in home settings.

Responsibilities

• Conduct comprehensive head-to-toe assessments and identify subtle changes in patient conditions
• Manage acute and post-acute care needs such as CHF, COPD, wound care, IV therapy, and post-surgical care
• Triage and manage urgent clinical situations independently in home settings
• Perform medication reconciliation, patient teaching, and safety monitoring
• Carry out wound assessments, care, and documentation

Requirements

• Current nursing license in good standing in Vermont
• At least one year of recent hands-on experience with adult/geriatric populations in settings like acute care, emergency, home health, ICU, or rehab

Skills

• Ability to work independently, make real-time clinical decisions, and prioritize urgent visits
• Strong problem-solving skills, attention to detail, and adaptability to varied environments
• Comfort with technology and efficient documentation practices
• Demonstrates cultural sensitivity and respect for patient autonomy
