About the Role
Join our elite team of healthcare professionals at Elara Caring, where you’ll play a key role in delivering high-quality home health care to over 60,000 patients daily. As a Registered Nurse Home Health, you’ll provide compassionate, autonomous care in patients’ homes, supporting the McHenry, IL area. This position includes a $5,000 sign-on bonus.
Responsibilities
- Align all activities with Elara Caring’s vision and leadership directives.
- Maintain patient and staff privacy and confidentiality in compliance with HIPAA.
- Assess assigned patients and families to identify physical, psychosocial, and environmental needs through documentation, clinical records, and evaluations.
- Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.
- Gather data on physical, psychological, social, and spiritual factors to incorporate into the plan of care.
- Conduct initial and follow-up evaluations, adjusting care plans as needed based on patient, family, or facility requirements.
- Communicate significant findings, health changes, or unsafe conditions to the Clinical Supervisor, physician, and other care team members.
- Revise the plan of care in response to patient needs and notify relevant team members.
Requirements
- Current, unrestricted state RN license.
- 1+ year of experience in a clinical care setting as a nurse.
- Able and willing to travel within the branch/office coverage area (50% travel required).
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
Qualifications (Preferred)
- Associate’s or Bachelor’s Degree in Nursing.
- Previous Home Health or Hospice experience.
- Experience with HomeCare HomeBase software.
You will report to the Clinical Manager or Clinical Supervisor.
Pay
The base salary for this position ranges from $83,000 to $95,000 annually, based on education, experience, skills, and internal equity.
