Registered Nurse - Home Care Resource
About the role
The Home Care registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice. The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting. The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.
Responsibilities
- Assess and evaluate patient care needs

- Prepare patients and their families for self-directed and/or assisted care
Requirements
- Baccalaureate degree in Nursing required
- Work experience as a registered nurse for at least two years
- Current license to practice as a professional nurse in Minnesota
- Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained
Qualifications
- Must be able to function independently with minimal supervision
- High level of sensitivity, assessment skills, technical-skills, and communications skills
- Demonstrates initiative, judgment, critical thinking, and the ability to problem solve
- Knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients
