About the role
Provides skilled nursing care in patients' homes, managing multiple cases with varying acuity levels.
Responsibilities
- Conducts admissions, assessments, recertifications, and discharges
- Administers IV therapy, dressing changes, and patient teaching
- Ensures safety with needs like lifeline and meal programs
- Documents care using Epic system
- Performs duties following policies and standards
Qualifications
- RN licensure
- Graduation from an accredited nursing program
- Valid driver's license with insurance
- Minimum three years of nursing experience, including at least one year in a hospital or home care setting
- CPR certification biannually
- Ability to lift 50# and transfer patients up to 100#
- Capable of bending, reaching, climbing stairs, and driving in various conditions
- Strong communication skills
- Ability to handle stress
- Ability to learn CMS regulations
