Responsibilities
- Patient Care & Advocacy
- Initial Contact & Onboarding
- Develop tailored plans of care
- Track patient progress
- Report changes in condition to physicians and care teams
- Teach or supervise household members assisting with patient care
- Team Leadership & Supervision
- Guide caregivers
- Monitor caregiver documentation
- Provide feedback
- Conduct supervisory visits
- Lead or oversee training sessions
- Ensure compliance and quality care
- Community & Growth Partnership Building
- Establish and maintain relationships
- Represent Care Advantage in the community
- Advocate for patient care improvements
- Aid in organizational growth
Requirements
- Active RN license in Virginia (compact/multi-state accepted)
- Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable)
- Associate's Degree in Nursing or higher
- Strong communication and presentation skills
- Ability to work collaboratively across teams
- Reliable transportation for home visits (mileage reimbursed)
- Comfortable with technology (computer/iPad documentation)
- Willingness to travel regularly within assigned area
