Role Overview

The Registered Nurse provides comprehensive nursing care in patients' homes, coordinating with healthcare teams to ensure continuity of care and developing individualized care plans.

Responsibilities

• Deliver high-quality clinical services within scope and infection control standards
• Coordinate patient care from admission to discharge with team members
• Conduct clinical assessments aligning with program and payer requirements
• Verify patient eligibility and medical necessity for services
• Develop, revise, and implement personalized care plans with healthcare providers

Qualifications & Skills

• Unrestricted RN licensure in the practice state
• Valid driver's license, vehicle insurance, and access to transportation
• Current CPR certification
• Ability to adapt to diverse home environments regardless of client background
• Preferred: 1+ years RN experience, independence, strong communication, writing, and organization skills
