Responsibilities

• Work with patients throughout their healthcare journey, guiding them through complex health concerns and helping them achieve their health and wellness goals.
• Identify and prioritize patients' needs, building comprehensive care plans that address Social Determinants of Health (SDOH), such as food resources, transportation access, and support at home.
• Build strong, trusting relationships with patients, focusing on empathy and active listening.
• Develop and implement patient care plans that holistically address social determinants of health.

Requirements

• Registered Nurse (RN) license in good standing.
• Experience working with diverse patient populations.
• Empathy and a bias toward action: a deep concern for people and a drive to act on behalf of those who cannot advocate for themselves.
• Clinical and organizational skills: ability to manage multiple tasks under pressure and balance clinical responsibilities with administrative duties.
• Technical adaptability: quick learning and efficient use of new systems and tools.

Qualifications

• Based in the United States.
