Registered Nurse (Full Time-Day) - Baton Rouge Rehab Hospital

JOB FUNCTIONS

• Captures patient health data and prioritizes assessments based on immediate needs.
• Conducts comprehensive physical and psychosocial assessments, involving patient, significant others, and health care providers as needed.
• Identifies and coordinates further assessments as necessary, ensuring triggers are completed.
• Determines nursing diagnoses based on assessment data and identifies expected outcomes.
• Develops and updates an interdisciplinary plan of care, integrating various assessments and current nursing practice standards.
• Collaborates with the interdisciplinary team to deliver direct patient care, adhering to nursing practice standards and universal precautions.
• Maintains patient rights, ensures timely treatments and medications, delegates tasks appropriately, coordinates care from support services, and provides patient education.
• Evaluates patient progress towards outcomes, revising diagnoses, outcomes, and plans as needed, and involves the patient and significant others in the evaluation process.
• Documents all treatments, medications, patient education, and assessments.

Requirements

• One year of nursing experience preferred.
• Louisiana Nursing License.
• CPR certified.
