Key Responsibilities
- Travel to pediatric patients’ homes to complete RN supervisory visits in accordance with payer and agency requirements.
- Complete and update CMS-485/Plans of Care and other required clinical documentation for pediatric home care cases.
- Perform focused nursing assessments and document skilled needs, progress toward goals, and any changes in condition.
- Review charts in the office to ensure documentation is complete, accurate, and compliant with regulations and payer standards.
- Collaborate with coordinators, administrators, and the clinical team to resolve issues, support staffing, and ensure continuity of care.
- Communicate with physicians, case managers, and other providers regarding patient status, care needs, and plan-of-care changes.
- Provide education and support to caregivers and families on the child’s care needs, equipment, and safety.
- Offer clinical guidance to direct care workers/home care aides working with pediatric clients.
Qualifications
- Active Registered Nurse (RN) license in good standing in the state of Pennsylvania.
- Pediatric experience and/or home care experience preferred (hospital, clinic, or home health/hospice all a plus).
- Valid driver’s license, reliable transportation, and willingness to travel to patients’ homes within the service area.
- Strong documentation skills and comfort using EMR/electronic documentation systems.
- Ability to work independently in the field while also functioning as part of a team in the office.
- Excellent communication, organization, and time-management skills.
