About the Role

Hospice of South Texas is seeking a compassionate and skilled Registered Nurse (RN) to join our Inpatient Center team. This position provides direct skilled nursing care to hospice patients of all ages while coordinating care with the interdisciplinary team, patients, families, and referring agencies to ensure continuity and quality of care. This role is ideal for nurses with strong clinical judgment, excellent communication skills, and a passion for palliative and end-of-life care.

Responsibilities

  • Assess hospice patients to identify physical, psychosocial, and environmental needs
  • Develop, implement, and document individualized plans of care
  • Provide direct skilled nursing care in accordance with hospice standards
  • Coordinate care with the interdisciplinary team and referring providers
  • Provide clinical direction to Hospice Aides and LVNs
  • Communicate effectively with patients, families, and healthcare professionals
  • Promote hospice philosophy, agency policies, and quality standards
  • Maintain clinical competency and participate in professional development

Requirements

  • Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing
  • Current Texas Registered Nurse license
  • Valid Texas Driver's License
  • Minimum two years clinical RN experience preferred
  • Hospice or home health experience strongly preferred
  • Reliable transportation and current auto liability insurance

Skills & Competencies

  • Strong nursing skills consistent with accepted standards of practice
  • Excellent interpersonal and communication skills
  • Knowledge of palliation and end-of-life care preferred
  • Strong clinical assessment and critical thinking abilities

Work Environment

  • Potential exposure to blood, bodily fluids, and infectious diseases
  • Ability to travel locally as needed
  • Night shift with 12-hour shifts and rotating weekends
  • PRN emergency call as needed
  • Possible exposure to inclement weather during travel

Physical Requirements

  • Prolonged standing and walking
  • Ability to lift up to 50 pounds and assist with patient movement
  • Ability to respond quickly in emergent patient situations
