This position is based out of Premier Washington-Oaks Springs, 36 Old Hickory Ridge Rd, Washington, PA 15301, and will float to the greater region as needed.

About the role

DaVita Kidney Care is bringing industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through DaVita Skilled Nursing Dialysis Services. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week, placing a significant burden on patients and adding avoidable costs to the healthcare system. DSD Services teammates will have access to all DaVita’s capabilities and resources, working to develop the infrastructure and grow this new service line.

In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. DSD RNs troubleshoot and make sound decisions related to dialysis treatment, occasionally partnering with SNF staff to coordinate care. The role includes supervising Dialysis Technicians delivering tandem care to meet program capacity and efficiency goals.

You will conduct impactful patient and family education in the SNF setting, deliver optimal dialysis care using best-practice techniques, and manage complex care with multitasking expectations. The role requires autonomy, critical thinking, and the ability to assess, diagnose, and implement comprehensive care plans.

Schedule

  • 3–4 days a week, 12+ hour shifts
  • No on-call requirement
  • Required travel to other SNF clinics within the region based on business and patient needs
  • Alternating Saturdays may be required

Training

  • Robust paid training program with a mix of classroom and hands-on learning
  • Dialysis experience preferred but not required

Responsibilities

  • Provide direct patient care, ensuring optimal dialysis treatment through best-practice techniques

Requirements

  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 12 months of RN experience
  • Associate Degree in Nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N) or a three-year diploma from an accredited program may be substituted
  • Basic computer skills and proficiency in MS Word and Outlook

Nice-to-haves

  • Previous chronic hemodialysis or acute dialysis experience
  • Training, teaching, or healthcare education background
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)
