About the role

The Registered Nurse (RN) at Ali Forney Center provides and coordinates care for LGBTQ homeless youth ages 16-24 in an ambulatory care setting. Responsibilities include care coordination, access to PreP and PEP, and plans for patients aging out of care.

Responsibilities

  • Care coordination with AFC staff
  • Coordination of access to PreP and PEP
  • Coordination of plans for patients aging out of care

Requirements

  • Demonstrates ability to perform clinical/technical/service/administrative tasks
  • Records, reports, and interprets clients' responses and collaborates with appropriate members of the health team
  • Systematically and continuously assesses patient's physical, psychosocial, and spiritual health status and knowledge base
  • Trades all sick and walk-in patients
  • Establishes therapeutic and supportive relationships with patients and families
  • Maintains an environment conducive to meeting patient care needs and facilitating patient flow
  • Promotes and provides for patient safety in accordance with state and federal guidelines
  • Practices and monitors infection control
  • Maintains patient flow and reassigns staff as needed

Qualifications

  • Education: AAS/BS in Nursing
  • Experience: Minimum of one (1) year experience in an Ambulatory/Medical Surgery/ER setting preferred
  • Bilingual, Spanish preferred
  • Experience: Demonstrated ability to plan, make decisions, organize, and direct the work of others
  • Nursing Experience: Minimum of two (2) years nursing experience
  • Skills: Demonstrated ability to perform venipuncture and administer medications including immunizations, demonstrated clinical, communication, interpersonal, and organizational skills, demonstrated ability to problem solve using critical thinking, demonstrated basic computer skills including typing
  • Licenses/Certifications: Licensed Registered Nurse in New York State, Current RN Registration Certificate, Current BCLS, Current Infection Control Certificate, Current Child Abuse Certificate
