Registered Nurse - Contract Nursing Home Liaison

About the role

The Marion VA Health Care System is seeking a full-time Registered Nurse to serve as the Contract Nursing Home (CNH) Liaison within Care Management & Social Work service located in Marion - IL.

Responsibilities

• Coordinates care focused on patient education, self-management, and customer satisfaction throughout the continuum of care
• Provides competent evidence-based care to assigned patients
• Manages, care coordinates, and discharges/disposes of patients in inpatient and outpatient settings
• Collaborates with multiple providers across all levels and sites of care
• Reviews medical records of patients hospitalized in non-VA hospitals to determine point of stability and potential for transfer to VA facilities
• Demonstrates knowledge of utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines
• Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others

Requirements

• Basic Language Proficiency
• Graduate of a school of professional nursing approved by an accrediting agency
• Current full, active, and unrestricted registration as a graduate professional nurse in a state, territory, or commonwealth of the United States, or the District of Columbia
• Current Basic Life Support (BLS) certification

Qualifications

• Education:
  • Nurse I - Level I: Associate Degree (ADN) or Diploma in Nursing
  • Nurse I - Level II: ADN or Diploma in Nursing AND 1 year of specialized nursing experience
  • Nurse I - Level III: ADN or Diploma in Nursing AND 2 years of professional nursing experience OR BSN and 1 year of professional nursing experience
  • Nurse II: BSN with 2 years of professional nursing experience OR MSN with 1 year of specialized nursing experience OR Master's degree in a related field with a BSN and 1 year of specialized nursing experience
  • Nurse III: MSN and 2 years of specialized nursing experience OR Master's degree in a related field with a BSN and 1 year of specialized nursing experience OR Doctoral degree in Nursing with 1 year of specialized nursing experience OR Doctoral degree in a related field with a BSN and 1 year of specialized nursing experience
• Experience:
  • Two (2) years of experience with Geriatric population - Nursing Home experience - experience with Case Management preferred
