Registered Nurse - Community Living Center Minimum Data Set
About the role
The Sioux Falls VA Health Care System seeks a Community Living Center Minimum Data Set Registered Nurse to support Veterans through resident-centered assessments, individualized care planning, documentation review, and interdisciplinary coordination.
Responsibilities
- Coordinate comprehensive MDS/Resident Assessment Instrument assessments that support individualized, resident-centered care planning for Veterans in the Community Living Center
- Develop and establish interdisciplinary treatment plans that address each resident's physical, psychological, social, spiritual, and emotional needs
- Ensure medical record documentation supports accurate MDS/RAI coding and reflects the resident's condition, preferences, and care needs
- Evaluate the effectiveness of care interventions and document observations, summaries, and assessment findings in the clinical record and RAI/MDS process
- Apply knowledge of Centers for Medicare & Medicaid Services long-term care standards to support compliant, high-quality assessment and care planning practices
- Collaborate with patients, families, peers, providers, nurse managers, administrators, and other health care practitioners to resolve issues impacting resident care
- Participate in inter-rater reliability activities to support the accuracy and consistency of RAI-MDS assessments
- Contribute to quality assurance and improvement activities, including SAIL and quality measure initiatives, to enhance care delivery and workflow within the Community Living Center
Requirements
- Basic English Language Proficiency
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE
- Current - full - active - and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia
- Graduate Nurse Technician (GNT) Exception: Candidates who meet the basic education requirements but do not possess the required licensure may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis)
Qualifications
- Basic Education Requirements: Associate Degree (ADN) or Diploma in Nursing, or Bachelor of Science in Nursing (BSN), or Master's degree in Nursing (MSN), or Master's degree in a related field with a BSN, or Doctoral degree in Nursing, or Doctoral degree in a related field with a BSN
- Specialized Experience: 1 year of specialized nursing experience equivalent to Nurse I - Level 1, or 2 years of professional nursing experience equivalent to Nurse I - Level 2, or 1 year of specialized nursing experience equivalent to Nurse I - Level 3, or 2 years of specialized nursing experience equivalent to Nurse II, or 1 year of specialized nursing experience equivalent to Nurse II

Pay

Schedule
Monday - Friday, 7:30am - 4:00pm
