About the role
The Registered Nurse - Community Health will use their training, critical thinking, and kindness to consider patient cases, identify solutions or suggest alternate approaches to patient problems. Much of the interaction will be over the phone, but the role is open to meeting patients in various settings.
Responsibilities
- Initiate care plans and manage/coordinate care
- Identify and address barriers affecting patients' ability to adhere to healthcare plans
- Build trusting relationships with patients to facilitate access to health care services and meet health needs
- Aid patients and families with navigating Neighborhood’s services as well as outside community resources, programs or services
- Attend community events as an RN to provide health education
- Participate in some evening street/shelter outreach, getting to know potential patients, assessing needs, and encouraging them to establish care
Requirements
- Current NYS Professional Registered Nurse license
- 2+ years clinical nursing experience
- CPR certification
- Knowledge of case/care management principles
- Attention to detail, organizational skills, project and time management skills, and ability to follow through

- Knowledge of Microsoft Office Suite, Adobe, and database management
Qualifications
- Degree from accredited two year nursing school

Monday-Friday
