Registered Nurse - Community Health
About the role
We are seeking a Registered Nurse passionate about providing high quality primary healthcare to high risk populations, including those in our Healthcare for the Homeless program. Essential duties include:
Responsibilities
- Initiate care plans, and manage/coordinate care; identify and address barriers affecting patients’ ability to adhere to healthcare plans
- Build trusting relationships with patients to facilitate access to health care services and meet health needs
- Auxiliary patients and families with navigating Neighborhood’s services as well as outside community resources, programs or services
- Attend community events as an Registered Nurse to provide health education
- Participate in some evening street/shelter outreach, getting to know potential patients, assessing needs, and encouraging them to establish care
Requirements
- Degree from accredited two year nursing school
- Current NYS Professional Registered Nurse license
- 2+ years clinical nursing experience
- CPR certification
- Knowledge of case/care management principles
- Attention to detail, organizational skills, project and time management skills, and ability to follow through

- Knowledge of Microsoft Office Suite, Adobe, and database management
Qualifications
The ideal candidate will have a Bachelor’s degree in Nursing or a related field, and a current NYS Professional Registered Nurse license.
