About the role

The Clinical Consultant provides clinical oversight of BLEHEALTH CHWs, lead care managers, Home Care aides, and housing navigators, ensuring they address medical and behavioral health needs and social determinants of health.

Responsibilities

  • Communicate with Case Managers and patients via phone or in-person meetings to discuss medication management, follow-up with providers, and manage emergencies.
  • Collaborate with interdisciplinary teams to advocate for clients and ensure they receive necessary services and progress towards care plans.
  • Develop and approve patient-centered care plans, including comprehensive assessments, health risk assessments, and reviews of medical visit summaries.
  • Provide health and preventive care education, engage vulnerable populations through home visits, accompaniment to appointments, and outreach to various settings.
  • Evaluate outcomes from targeted interventions using data, make adjustments to care plans, and expand programming addressing social determinants of health.
  • Coordinate hospital admission/discharge plans, perform medication reconciliation, and support medication adherence.
  • Implement prevention and engagement activities, participate in quality improvement efforts, and manage service delivery effectiveness.
  • Monitor ongoing client care coordination, document assessments, care plans, referrals, outcomes, and progress notes, ensuring timely and high-quality documentation.
  • Train, monitor, and support staff to provide appropriate services, ensuring ongoing achievement of quality measures.
  • Maintain communication with managed care providers and other contractual partners, provide updates, and ensure timely outreach to eligible members.
  • Analyze and identify tailored solutions for member-specific needs, aligning with unique requirements.

Requirements

  • Registered nurse with current licensure to practice professional nursing in California.
  • Bachelor’s degree in Nursing from an accredited program by the National League for Nursing, or Master’s degree in health care preferred.
  • Three years of management experience within the last five years in a home health or related health care organization, with at least one year in a supervisory or administrative capacity.
  • A registered nurse with four years of experience within the last five years in a home care agency, primary care clinic, or health facility, with at least one year in a supervisory or administrative capacity.
  • Demonstrated ability to supervise and direct professional administrative personnel.
  • Knowledge and expertise in clinical decision-making and Quality Management review and evaluation.
  • Ability to market and deal tactfully with customers and the community.
  • Licensed driver with an insured vehicle in good condition.
  • Excellent observation, verbal, and written communication skills.
  • Knowledge of business management and governmental regulations.
  • Participation in Quality Assurance/Performance Improvement Program.
