About the role
Lead the implementation and continuous monitoring of personalized patient care plans to ensure every individual receives treatment tailored to their specific recovery goals. Coordinate multidisciplinary teams as the primary point of contact, ensuring seamless communication across all clinical specialties for holistic patient support. Advocate for patient safety by diligently executing physician orders and maintaining precise records of the care journey to uphold the highest standards of quality. Assess and facilitate comprehensive discharge strategies, collaborating with the healthcare team to ensure patients have a safe, well-supported transition home.

Requirements
- Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
- Diploma from an accredited school/college of nursing required OR a bachelors level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.
Preferences
- Requires 1–2 years of Registered Nurse (RN) experience, with specific background in Obstetrics (OB) and Gynecology.
- Prior experience in Labor and Delivery (L&D) is highly preferred.
