Schedule

Summa Hospice Schedule: M-F 8A-4:30P Rotating weekends and holidays. Territory: Akron, Richfield, Copley, Fairlawn, Medina areas.

Responsibilities

• Oversees the overall care of the patient from beginning to end of life
• Does initial, comprehensive, and ongoing skilled nurse visits for patient and family as determined by plan of care
• Updates all doctor orders in patient chart accordingly
• Notifies doctor of any significant changes of the patient’s condition
• Manages interdisciplinary care as applicable
• Provides end of life care in a patient’s home as ordered by the attending physician
• Observes and monitors patient conditions
• Individualizes plan of care and adjusts as needs change
• Administers medication as prescribed by the physician
• Promotes continuity of care with appropriate admissions, transfers and discharges
• Counsels patient and family on the disease
