Registered Nurse Case Manager (RN)

Responsibilities
- Respond to clinical referral information with care and expertise, addressing inquiries and requests compassionately.
- Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
- Provide direct nursing care as an RN Case Manager, ensuring patient comfort and quality of life.
- Deliver top-notch nursing care rooted in proven principles and techniques to ease pain and bring solace.
- Educate and empower patients and families, encouraging their active participation in personalized care plans.
- Participate in call duties to ensure support is available when it matters most.
Requirements
- Current unencumbered license as a Registered Nurse (RN).
- Minimum one (1) year of experience as an RN in a medical-surgical or acute care setting.
- Hospice experience is a plus.
- Computer proficiency, including experience with EMR systems.
- Current CPR certification (if required by state).
- Valid driver’s license, vehicle insurance, and access to dependable transportation.

Pay
$31.00 - $36.00 per hour. The pay range listed is a general guideline; final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position.
