Registered Nurse Case Manager - Hospice

About the role

Hospice of the Midwest is seeking a passionate, dedicated Hospice RN to join our team in Woodbury, MN. Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. The ideal candidate will provide 1:1 care, benefit from flexible scheduling, and enjoy the opportunity to work independently while also having team support.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Pain and symptom management
• Education and support for the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Pay

USD $90,000.00 - $95,000.00 / Year

Schedule

Mon-Fri, 8:00 AM - 5:00 PM
