Registered Nurse Case Manager - Homes St. Lucie County

Sign-on bonus: $4,500. Schedule: Monday – Friday, 8:00 AM – 4:30 PM.

About the Role

The Registered Nurse (RN) is responsible for planning and delivering patient care, providing skilled nursing care and oversight in accordance with policies and the Florida Nurse Practice Act. This role utilizes the nursing process of assessment, planning, intervention, implementation, and evaluation to provide a combination of skilled professional nursing tasks, palliative, and supportive care to patients and their families.

Responsibilities

• Provide program development strategies for Treasure Coast Hospice in an organized, routine capacity to enable team cohesion.
• Work collaboratively with other departments, demonstrating leadership for a comprehensive approach to program development.
• Inspire staff to live Treasure Coast Hospice’s Mission, achieve its Vision, and demonstrate its Values and Commitment.
• Stay current in legal and compliance hospice issues.
• Uphold Treasure Coast Hospice/Model Standards for patient visits and compliant documentation.
• Perform comprehensive assessments including physical, emotional, spiritual, and psychosocial needs of patients and families.
• Establish and update a plan of care based on goals of care, collaborating with the interdisciplinary team (IDT), patient, family, and attending physician.
• Provide direct patient care, including skilled nursing tasks such as assessments, medication administration (PO/SL, Subcue, IV, PR), wound care, and activities of daily living (ADLs).
• Administer medications according to policy, evaluate effectiveness, and document accordingly.
• Manage controlled substances per physician orders and policy, including proper disposal of unused medications.
• Communicate pertinent patient and family information to the IDT and actively participate in plan-of-care reviews.
• Document variances in patient condition, interventions, outcomes, and progress toward goals.
• Provide and document patient and family education, including their understanding of the topics taught.
• Supervise Certified Nursing Assistants (C.N.A.s) and Licensed Practical Nurses (LPNs).
• Maintain credentialing requirements for healthcare facilities and hospitals.
• Support the Treasure Coast Hospice compliance program, adhering to all federal, state, and local laws.
• Fulfill responsibilities related to the organization’s strategic plan as assigned.

Requirements

• Current and active Florida RN license.
• Must obtain Certified Hospice and Palliative Registered Nurse (CHPN) certification within 12 months.
• Current and valid Florida driver’s license and automobile insurance.
• Current CPR certification.
• Minimum of one year of RN experience where physical assessment skills were utilized (preferred).
• Prior experience in oncology, home health, hospice, or palliative care (preferred).
• Ability to handle sensitive issues with sound judgment, maintain professional boundaries, and ensure confidentiality.
• Strong organizational, planning, and prioritization skills.
• Self-directed and able to work independently in the field.
• Basic computer skills and experience with Electronic Medical Records (EMR) software (preferred).
• Regular attendance is a condition of employment.

Working Conditions

• OSHA Category I (routine exposure to blood and body fluids/tissues).
• Physical demands include:
  • Standing, sitting, walking.
  • Lifting up to 50 lbs without mechanical aids.
  • Pushing, pulling, carrying objects.
  • Climbing stairs or ladders.
  • Crouching, kneeling, crawling, stooping, reaching, handling objects.
  • Frequent use of arm and leg muscles.
  • Transferring patients (e.g., bed to chair, chair to toilet).
• May be required to perform emergency duty before, during, or beyond normal work hours in crisis situations or disasters, including at evacuation sites.
• Work location is smoke-free; adherence to smoking restrictions is expected.
