About the Role

We are seeking a dedicated Registered Nurse (Home Health) to provide personalized, in-home nursing care that promotes patient recovery, safety, and independence.

Responsibilities

• Serve as the primary clinical partner for patients and families
• Perform comprehensive in-home assessments and develop individualized care plans
• Administer skilled interventions such as wound care, infusions, medication management, and post-operative care
• Educate and empower patients and caregivers for symptom and medication management
• Monitor patient progress, identify early changes, and communicate with care teams
• Coordinate care across providers to ensure continuity and quality outcomes
• Maintain accurate, timely documentation to support compliance and quality measurement

Requirements

• Active RN license in the practice state
• At least one year of clinical nursing experience
• Patient-focused, organized, and motivated to deliver high-quality care
• Valid driver’s license and reliable transportation
• CPR certification or ability to obtain
