About the Role
We’re looking for a compassionate and motivated Registered Nurse Case Manager to join our team, supporting members in the Danville & Martinsville, Virginia area. This is a field-based role that blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care.
Responsibilities
- Conduct comprehensive assessments (telephonic and face-to-face) to evaluate members’ medical, behavioral, and social needs
- Develop and implement personalized care plans that promote both short- and long-term wellness
- Coordinate care across providers, programs, and services to ensure a seamless experience for members
- Use clinical expertise and data insights to identify risks, gaps in care, and opportunities for intervention
- Support members with complex or multiple conditions by applying a holistic, whole-person approach
- Facilitate referrals to appropriate clinical and community resources
- Participate in case conferences and collaborate with multidisciplinary teams to optimize care strategies
- Review claims history and benefit eligibility to inform care planning
- Engage members using strong interviewing skills to build trust and encourage active participation in their care
- Ensure all case management activities comply with regulatory and organizational guidelines
Requirements
- Active, unrestricted RN license in Virginia
- At least 3 years of clinical experience (hospital, home health, ambulatory care, or similar settings)
- Residence in Danville & Martinsville and in-between communities in Virginia
- Comfort with frequent local travel and in-home visits
- Strong assessment, communication, and critical-thinking skills
- Ability to work independently and manage a flexible daily schedule
Preferred Qualifications
- Bilingual (English/Spanish)
- Case management or care coordination experience
- Managed care environment experience
- Discharge planning or transition-of-care experience
- Crisis intervention skills
Education
- Associate Degree in Nursing (required)
- Bachelor of Science in Nursing (BSN) (preferred)
Schedule
Monday–Friday, 8:00 AM – 5:00 PM EST
Anticipated weekly hours: 40
Time type: Full time
Pay
The typical pay range for this role is $60,522.00 - $129,615.00. This pay range represents the base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
