Faith Hospice is seeking a Registered Nurse (RN) Hospice Case Manager who shares a passion for serving patients at the end of life and working as part of an interdisciplinary team.

What Will You Do?

• Work a Monday - Friday daytime schedule
• Participate in a back-up on-call rotation with other RNs on the team
• Manage a case load of hospice patients within an assigned geographic territory
• Spend more time with patients by minimizing drive time

Education And Experience

• RN licensure in Michigan
• At least 2 years of med/surg or acute care nursing experience
• Hospice and/or hospital experience preferred
