About the role

Join our Community Hospice team as a Case Manager Registered Nurse to provide compassionate, skilled in-home care, supporting patients and families in Chittenden and Grand Isle counties. As a vital member of the interdisciplinary team, you'll deliver nursing care, educate families, and create a supportive environment that respects patient wishes. This in-person position involves performing home visits and case management duties.

Qualifications

• Current RN licensure recognized by the State of Vermont.
• Minimum of 2 years' experience in home health, hospice, or med-surg settings.
