Registered Nurse/Case Manager

Key Responsibilities

• Cook up and oversee patient care, maintaining the highest standards.
• Create, execute, and assess personalized patient care plans.
• Collaborate with interdisciplinary teams to provide comprehensive care.
• Inform patients and their families about treatment plans and health strategies.
• Keep detailed and timely records of patient care activities.
• Investigate and implement improvements to enhance patient outcomes.

Qualifications

• Current and valid RN license.
• Experience in case management or a related field is preferred.
• Strong organizational and communication skills.
• Ability to work independently and as part of a team.
• A commitment to delivering compassionate and patient-centered care.

Schedule

Weekend Requirement - typically one full weekend per 4 weeks, subject to departmental needs.
