Registered Nurse – Case Manager

About the role

The Registered Nurse serves as the case manager for assigned patients and as the teacher for patient’s caregiver(s). The RN case manager utilizes the nursing process and the six phases of the Alive CARE visit to provide safe, end of life care to the to the terminally ill patient/family unit. The primary responsibility of the RN case manager is to coordinate and provide hospice care for home care patients through the development, initiation and updating of a plan of care in collaboration with patient and family.

Requirements

• Educational Qualifications: Registered Nurse with a current nursing license, BSN preferred.
• Experience: Minimum one-year nursing practice or equivalent experience required. Hospice, Oncology, or Home Health and/or long-term care preferred.
