About the role

The Registered Nurse Case Manager is responsible for managing patient care plans, coordinating services, and ensuring quality outcomes. This role requires excellent communication skills and the ability to work effectively in a team environment.

Responsibilities

• Develop and implement care plans based on patient needs
• Coordinate services between healthcare providers and other agencies
• Monitor patient progress and adjust care plans as necessary
• Communicate regularly with patients and families to ensure their satisfaction and understanding of their care plan
• Ensure compliance with regulatory standards and policies

Requirements

• Bachelor's degree in Nursing (BSN)
• Current RN license in Colorado
• At least 2 years of clinical nursing experience
• Experience in case management or related field preferred

Qualifications

• Strong organizational and time management skills
• Excellent interpersonal and communication skills
• Ability to work independently and as part of a team
• Proficient in Microsoft Office Suite
