About the role

This PRN day shift opportunity plays a key role in supporting patients throughout their hospital stay by ensuring coordinated, high-quality care as part of our Case Management team.

Responsibilities

• Provide comprehensive care coordination for hospitalized patients.
• Conduct high-risk screenings and assessments to identify patient needs.
• Arrange and document post-acute care services, ensuring smooth transitions of care.
• Collaborate with a multidisciplinary healthcare team to support optimal patient outcomes.

Requirements

• 1 year of case management experience
• Associate of Science in Nursing (required)
• Bachelor of Science in Nursing (preferred)
• Basic Life Support (BLS) certification (required)
• Florida Licensed Registered Nurse (required)
